Small Business Assistance Program Award Nomination Questions 

*Please note that questions 6 through 10 below were changed on November 18, 2005.  Please use these questions for the nomination application.

The nominator should address the following points, where applicable, in the body of the summary:

  1. Describe the leadership abilities of the program or individual nominated.  Describe how these leadership abilities have advanced the goals of the SBO/SBAP.
     

  2. Has the program or individual served as a role model for their peers?  Explain.
     

  3. Describe involvement of the individual or program with the National Steering Committee (NSC) and/or state program activities that support the goals of the SBOs/SBAPs.
     

  4. Describe the committees, meetings, and other activities that the individual or program has been involved in and how they have advanced the goals of the SBOs/SBAPs.
     

  5. What activities or products has the individual or state program developed to assist small businesses that has been made available or shared with other programs?  What were the timeframes: one-time event, periodic, ongoing?  Please describe.
     

  6. Describe the role the individual or program played to position the NSC to more effectively achieve its goals.  Be specific; show the linkage to a goal.
     

  7. Did the individual or program create or help to create any in-state, regional, or national partnerships?  Please describe.
     

  8. Has the individual or program created or helped to create a unique work product, outreach, or research for a 507 program or programs?  Please describe and be specific.
     

  9. Did the individual or program provide leadership in the execution of a workshop, summit, subcommittee, or other SBO/SBAP activity on a national, regional, or local level?  Please describe and be specific.
     

  10. How has the individual or program promoted or increased awareness of the SBO/SBAP or the NSC?  Be specific.  Tell us how it was exceptional.
