Small Business Recognition Awards: 2013 Online Nomination Form

Instructions and Submittal Deadline
This online nomination form should be submitted electronically.  Responses must adhere to the word limits listed for each question.  Supplemental attachments are limited to a total of 3 pages that stand alone and do not require the committee to read all of the referenced web pages in order to understand the nomination.  If you cannot submit electronically or prefer to mail or fax hard copy materials, you may download the application to print out HERE.  Please mail or fax to Phyllis Copeland at the address/fax below.

This online form and all accompanying materials must be submitted by 4:00 pm EST, Friday, March 22, 2013. If you are submitting materials by mail or fax, the hard copy application (including up to the word limits listed for each response), and any supplemental information (up to 3 pages total) must be RECEIVED by Phyllis Copeland by the same date, March 22, 2013.  Please contact Phyllis Copeland (below) with any questions.
