Please provide responses of up to 200 words addressing each of the questions outlined for the Small Business Award of interest.  It is recommended that you develop the responses in Word or another program with word counting capabilities.  Mail or fax this hard copy form, activity summary, and any supplemental information (up to 3 pages total) together to Phyllis Copeland (above).  Note that the entire application will be disqualified if the word limits (200 words each question) or supplemental information limits (3 pages) are exceeded.  Please provide specific descriptions and examples to support each of your responses.  Any attachments sent must be in support of one of these questions.
